CMS announced September 24 that nearly $23 million in Rural Health Transformation Program funding will support Delaware’s three federally qualified health centers. The award includes electronic-record upgrades, remote patient monitoring, cybersecurity, expanded care teams, care coordination, two mobile health units and a passenger van.
Those projects do not automatically create a smooth handoff to the home. Mobile screenings, monitoring alerts and referrals still need a clear destination, an accountable owner and a fast response.
DomAfina’s take: home-care provider organizations serving rural communities should identify FQHC partners now, agree on a minimum referral dataset, assign after-hours alert ownership and test a closed-loop referral from mobile unit to first home visit. The opportunity is not simply more technology; it is a reliable path from a new point of access to sustained care at home.
