CMS’s 2027 home health proposal reaches beyond reimbursement. It would strengthen denial and revocation authority when owners or operators are tied to noncompliance, fraud, waste or abuse.

That makes the ownership record a living compliance file—not paperwork touched only during enrollment. Providers should be able to show who controls the organization, when a change occurred, which credentials and exclusions were checked, and who approved the update.

The warning sign is not always a bad claim. Sometimes it is a stale address, an unrecorded ownership change or a credential that expired quietly. Put those records on a monitored workflow before an auditor asks for them.
