Reported October 23, 2026: CMS and states announced a $275 million Nurses for Nursing Homes initiative. Eligible RNs and LPNs/LVNs may receive up to $40,000 in student-loan repayment and/or a $10,000 stipend for a three-year commitment at a qualifying rural or high-shortage nursing home. All 50 states and D.C. are participating; applications open October 26. The program requires at least 30 hours weekly and does not fund home health agencies.
DomAfina’s take: treat this as a new competitor for the same nurses. Within seven days, identify clinicians with student debt, long routes or unstable schedules and hold individual stay conversations. Compare total compensation, mileage, weekend burden, documentation time, schedule control and advancement—not hourly pay alone. Record each nurse’s top two reasons to stay or leave, then assign an owner and deadline for each fix. A retention response started now is cheaper than reacting after a three-year incentive wins the decision.
