CMS is proposing closer alignment between the expanded Home Health Value-Based Purchasing Model and the Home Health Quality Reporting Program. The direction is clear: quality data should tell one coherent story.
Providers should stop treating the two programs as separate reporting seasons. Build one scorecard that identifies the measure, reporting window, source record, responsible owner and action taken when performance slips. Then make every number traceable to the patient-level evidence behind it.
A dashboard is useful only when a team can explain why a measure moved—and what it will do next. Alignment at CMS should trigger alignment inside the provider organization.
