On October 5, HHS and CMS finalized new Transparency in Coverage rules. Plans and issuers must publish cleaner network-level rate files, exclude implausible provider-service combinations, add taxonomy and utilization files, lower the out-of-network reporting threshold from 20 claims to 11, and update core files quarterly instead of monthly. Phone access to personalized cost-sharing information begins for plan years starting January 1, 2027.
DomAfina’s take: home care providers should treat this as contract intelligence, not payer paperwork. Assign an owner to map each payer network ID to your contracts, compare disclosed rates with remittances, and flag services whose billed, allowed and contracted amounts disagree. Feed the result to intake with a clear estimate disclaimer. Cleaner public data will not fix a bad contract—but it can make one harder to hide.
