Under CMS-0057-F, impacted payers must operate prior-authorization APIs by January 1, 2027. A response must approve, deny with a specific reason, or say what additional information is needed.

Home health providers may not own the payer API, but they live downstream from it. Referral packets, clinical documents and authorization status need structured fields that can move cleanly—not a fax attachment no one can search.

Map the handoff now: what arrives, who checks it, which fields are missing, and where the status is recorded. The winners in 2027 will not be the teams with the newest acronym. They will be the teams that stop retyping the same referral three times.
